Community Access

Community Access Services provides community-based supports with an emphasis on accessing community resources and building relationships as they participate in vatious activities. Individuals are supported with exploring their interests and with pursuing their goals around developing personal skills and abilities. The Community Access activities may have a recreation or volunteer component in the community. the focus of these supports is to promote full and inclusive lives for individuals, to assitit in developing cvalued societal rolesm and reflect the Pillars and Domains of the Quality of Life Framework: Well Being, Independance, Community Connectedness, and Quality of Services. Our stage takes the time to know each individual personally, ensuring that each person progresses as far and as fast as possible.

Supported Independent Living

The most independent people we serve live alone in their own homes or apartments and need only limeted suport in a few areas of life.

Edenbridge's friendly and helpful staff are hand-picked for their resonsiveness and sensitivity to the special needs of our independent adults. Supported Independent Living offers a vonvenient way to "make up the difference" in a discreet and caring way.

Supported Independent Living services are tailored on a strictly individual basis and there are virually no limits on what assistance can be provided. Plans are flexible, higly invovative, are developed strictly on an individual basis, and willreflect hte Pillars and Domains of Life Framework: Well Being, Independence, Community Connectedness, and Quality Services.

Overnight Staffed Residence

An Overnight Staffed Home is a choice for individuals requiring more care than can be provided in a Support Home. These arrangemnts offer up to 24 hour support in an environment that is considered to be their own home. Individuals receiving supports in thse settings will also be provided opportunities to make their own choices as well as haveing appropriate responsibilities, this environment encourages personal progress as well as building important community relationships. Thses servics will also be developed strictly on an individual basis and will reflect the Pillars and Domains of the Qulatity of Life Framework: Well Being, Indempendence, Community Connectedness, and Quality Services.

Our Overnight Staffed Residences offer 24 hour care in a warm and stable home environment, respect for each individual's like and dislikesm and the secure "at home" feeling that encourages personal progress. People in an Edenbridge group home always have lots of fun and receive plenty of personal attention.

Endenbridge ensures these supports and choices are taking place through monthly (or more frequent as needed) meeting with the individual and their support worker. Documentation of these meetings is kept on file, and guardians provided with monthly summary of services.

Respite

In-Home Respite

In-Home Respite is presently provided for indivduals who may require support while their family or primary support workr requires some time off from their regular schedule. These supports are an extension of the supports that an individual receives whiil in their support home. Individuals continue to receive individualized supports that provide opportunity for personal growth, to develop indepedent living skillsm encourage individual responsibility, as well as feedom to make choices.

Out-of Home Respite

Out-of Home Respite is presently provided for idividulas who may require occasional overnight support while their family or primary support worker requires som time off from their regular schedule. These supports are an extension of the supports that an individual receives while in teir support home. Individuals continue to receive individualied supprots that provide for the opportunity for personal growth, to develop independent living skills, encourage individual responsibilitym as well as freedome to make choices.

Support Homes

Individuals that may require supervision or some hands on support with their daily skills may choose a Support Family or Supporting Roommate setting. These supports are provided in a home where the living arrangements are shared between the support worker and the individual. A Support Home provides individualized servis that encourage individual resonsibility, freedom to make choicesm as well as the opportunity to develop independent living skills. Support Homes are chosed with a focus to developing positive community relationships and replect the Domains of the Quality of Life Framework: Well Being, Independence, Community Pillars Connectedness, and Quality Services.
